• TV, Theatrical, Net Code, New Media - Use this form when reporting earnings and contributions covered under the Television, Theatrical, Network Code and New Media contracts.

• Commercials -  Use this form when reporting earnings and contributions covered under the Commercials contract including Television, Audio and the Armed Forces contracts.

• Influencer, Cameo Agreement -  Use this form when reporting earnings and contributions covered under the Influencer or Cameo Agreements.

• Podcasts - Use this form when reporting earnings and contributions covered under the Independent Podcast Agreement.

• Co-Ed, Interactive, Radio, Audiobooks – Use this form when reporting earnings and contributions covered under the Corporate/Educational, Interactive, Radio Program and Audiobook contracts.* 

  *Reporters under the Sound Recordings Code should continue to use the forms provided within the collective bargaining agreement.

• W9 Form - Please note that even though the preparation date may not be current, this form is still valid for W-9 purposes.
• Dancer Choreographer Form - This form will need to be filled out if reporting earnings on behalf of Dance Choreographers and Assistant Choreographers under the Theatrical Agreement, Television Agreement, Commercials Contract, Network Code Agreement, or the Music Video Agreement.
• MSC Cover Sheet - Use this form when submitting multiple service commercial allocation earnings.
